Cats living in warm, humid climates where flea populations thrive year-round face constant exposure pressure. Cats in multi-pet households may have higher flea exposure if any pets inadequately protected from fleas. Shelter cats and cats from hoarding situations may have had heavy flea exposure that triggered sensitization. Young adult cats between one and five years of age most commonly develop FAD, though it can occur at any age. Mixed breed cats are just as susceptible as purebred cats, and FAD is common across all feline populations.

For cats at elevated risk of flea allergy dermatitis, proactive prevention measures are especially important. Starting flea prevention before sensitization develops is ideal for kittens and young cats. Selecting highly effective flea preventatives and maintaining strict year-round compliance provides the best protection. Cats adopted from situations where flea exposure was likely should be started on flea prevention immediately and monitored closely for signs of FAD. Owners of cats with other allergic conditions should be particularly vigilant, as these cats may be predisposed to developing FAD as well. Regular veterinary monitoring helps catch early signs of allergic skin disease before severe symptoms develop.

Several conditions commonly co-occur with flea allergy dermatitis or share underlying allergic mechanisms. Atopic dermatitis, caused by environmental allergen sensitivities to substances like pollens, molds, and dust mites, frequently occurs alongside FAD in cats with allergic tendencies. Food allergies may coexist with flea allergy, and some cats have all three types of allergic skin disease simultaneously, creating complex diagnostic and management challenges. Eosinophilic granuloma complex, including indolent ulcers, eosinophilic plaques, and linear granulomas, can develop in cats with FAD as another manifestation of the underlying allergic tendency. Cats with FAD may also be more prone to developing allergic respiratory conditions including feline asthma.

Several conditions produce clinical presentations similar to flea allergy dermatitis and must be differentiated through careful evaluation. Food allergy can cause identical skin lesions and distribution patterns, and distinguishing food allergy from FAD may require dietary elimination trials. Environmental allergies may produce overlapping symptoms, particularly in cats also affected by FAD. Cheyletiella mite infestation causes intense itching and skin lesions that can mimic FAD. Dermatophytosis occasionally presents with patterns resembling allergic skin disease and should be ruled out with fungal culture. Pemphigus foliaceus, an autoimmune skin condition, may produce pustular and crusty lesions. Psychogenic alopecia involves excessive grooming due to behavioral causes rather than physical pruritus.

Complications can develop when flea allergy dermatitis is inadequately controlled or left untreated. Secondary bacterial infections are common in cats with FAD, as traumatized skin from scratching provides entry points for bacteria normally present on the skin surface. These infections can progress from superficial pyoderma to deep infections or abscesses if not treated. Chronic self-trauma leads to permanent skin changes including scarring, hyperpigmentation, and lichenification. Hair loss may become extensive and may not fully regrow in severely affected areas. The development of eosinophilic granuloma complex lesions can complicate management and require additional treatment. Chronic stress from persistent itching can affect behavior and overall wellbeing.
