This condition occurs when bacteria, and less commonly fungi, colonize the disc space and surrounding vertebral bone, causing progressive destruction and inflammation. While relatively uncommon in cats compared to dogs, diskospondylitis represents a serious condition that can cause significant spinal pain, neurological deficits, and systemic illness if not diagnosed and treated appropriately. The infection can occur at any level of the spine but is most commonly found in the lumbar and lumbosacral regions. Cats of any age can be affected, though adult cats appear most commonly diagnosed.
The causes of diskospondylitis involve infectious organisms reaching the spine through the bloodstream from infections elsewhere in the body, direct extension from adjacent tissues, or introduction during spinal procedures or penetrating injuries. Bacterial infections account for the majority of feline cases, with organisms such as Staphylococcus, Streptococcus, and Escherichia coli frequently identified. Fungal causes including Aspergillus and other organisms occur less commonly but can be particularly challenging to treat. The source of infection is often not identified, though skin wounds, dental disease, urinary tract infections, and other systemic infections can serve as origins from which organisms spread to the spine.
The impact of diskospondylitis on affected cats can range from localized spinal pain to severe neurological compromise depending on the location and extent of infection. Cats typically show significant pain manifesting as reluctance to move, abnormal posture, and resistance to handling. As the infection progresses and causes vertebral destruction, spinal instability can develop, leading to spinal cord compression and neurological deficits. Systemic illness with fever, lethargy, and appetite loss often accompanies the spinal symptoms. Without treatment, the infection can spread to additional spinal segments, cause vertebral collapse, or seed other organs, leading to life-threatening complications.
Diskospondylitis in cats is treatable with prolonged antimicrobial therapy, though treatment requires significant time commitment and follow-up care. Early diagnosis before extensive vertebral destruction or neurological damage occurs improves prognosis considerably. Treatment typically extends for many weeks to months to ensure complete eradication of infection from the poorly vascularized disc space. Cats showing signs of spinal pain or neurological abnormalities should receive prompt veterinary evaluation to enable early diagnosis and treatment initiation. With appropriate therapy and monitoring, many cats with diskospondylitis can achieve resolution of infection and return to good quality of life.
