Capillariasis, also known as bladder worm infection, is a parasitic condition of the feline urinary tract caused primarily by Pearsonema feliscati (formerly classified as Capillaria feliscati), a small threadlike nematode that inhabits the urinary bladder and occasionally the renal pelvis of infected cats. This parasitic infection occurs worldwide but is relatively uncommon compared to other feline parasitic diseases, with highest prevalence in cats with outdoor access and hunting behaviors. The condition often goes unrecognized due to its nonspecific symptoms that overlap with more common urinary tract conditions.
Cats acquire bladder worm infection through ingestion of embryonated eggs from the environment or by consuming earthworms that serve as intermediate hosts. The eggs are remarkably resistant to environmental conditions and can remain viable for extended periods in contaminated soil. Following ingestion, larvae develop and migrate to the urinary tract where they mature into adult worms, embed in the bladder mucosa, and begin producing eggs that pass in urine. The complete life cycle from infection to egg-laying adults takes approximately two months, with adult worms surviving up to a year in the feline host.
The impact of capillariasis on feline health varies considerably based on worm burden and individual cat factors. Light infections may cause no apparent symptoms, with affected cats remaining completely asymptomatic carriers. Heavier worm burdens or sensitive individuals can develop clinical signs of lower urinary tract disease including increased urination frequency, straining, blood in urine, and inappropriate elimination. The presence of worms and eggs in bladder tissue causes inflammation that produces symptoms identical to more common urinary conditions, potentially leading to misdiagnosis if parasitic infection is not considered.
Capillariasis is readily treatable with appropriate antiparasitic medications, and most cats recover fully following treatment. Accurate diagnosis requires specific testing, as routine urinalysis may miss the characteristic eggs if not specifically sought. Veterinary evaluation is important both for confirming the diagnosis and ruling out concurrent urinary conditions that commonly coexist. Understanding the environmental transmission route helps cat owners implement preventive measures, particularly limiting exposure to potentially contaminated soil and earthworms in cats with outdoor access.
