Cloacal prolapse is a serious medical emergency in birds in which the inner tissues of the cloaca, the common chamber that serves as the exit point for the digestive, urinary, and reproductive systems, protrude outside the body through the vent. This condition occurs when the muscular support structures of the cloaca are overwhelmed by straining, weakened by chronic stress, or damaged by underlying disease, allowing the normally internal tissues to evert and become visible externally. Among pet birds, cockatoos and other larger parrot species are particularly susceptible to cloacal prolapse, especially females that experience chronic egg-laying or reproductive disorders. The condition requires immediate veterinary attention, as exposed internal tissues quickly become damaged, infected, and life-threatening if not properly treated.
The cloaca in birds serves as a multifunctional chamber where the digestive tract, ureters, and reproductive tract converge before exiting through the vent. During a prolapse, some or all of the cloacal wall may evert, and in severe cases, portions of the oviduct, intestine, or other internal structures may also prolapse through the compromised vent. The exposed tissues, designed to remain moist and protected inside the body, rapidly dry out, swell, and become vulnerable to trauma and infection when exposed to air and the external environment. Birds often pick at the abnormal tissue, causing additional damage and bleeding. Without rapid intervention, the prolapsed tissue can become necrotic, and the bird can die from shock, infection, hemorrhage, or obstruction of the digestive and urinary tracts.
Cockatoos face elevated risk for cloacal prolapse due to several factors related to their reproductive behavior and captive management. Female cockatoos in particular often develop patterns of chronic egg-laying when stimulated by environmental and social factors in captivity. Repeated straining to lay eggs, especially if eggs are abnormally large, malpositioned, or if the bird is calcium deficient, weakens the cloacal muscles over time. Additionally, cockatoos are prone to reproductive disorders including egg binding and oviductal disease that predispose them to prolapse. Obesity, which is common in pet cockatoos, adds to the mechanical strain on pelvic structures. Male cockatoos can also experience prolapse, though less commonly, often associated with cloacal papillomas or other masses, or with excessive straining from constipation or other causes.
With prompt, appropriate treatment, many birds can recover from cloacal prolapse and return to a good quality of life. Treatment typically involves gently cleaning and reducing the prolapsed tissue, addressing any underlying causes, and often surgical intervention to reinforce the vent and prevent recurrence. The prognosis depends heavily on how quickly treatment is initiated, the extent of tissue damage, and whether underlying causes can be successfully managed. Birds with chronic or recurrent prolapse face more guarded prognoses, and some may require aggressive interventions including spaying to eliminate the reproductive contribution to the problem. Owner awareness of this condition and its warning signs enables rapid response that can be life-saving for affected cockatoos.
