Conditions presenting with similar symptoms require careful differentiation from primary foot necrosis syndrome. Pododermatitis (bumblefoot) causes foot lesions, pain, and lameness but results from pressure sores and secondary infection rather than vascular compromise. Gout produces painful foot swelling and lesions due to uric acid crystal deposition, often in conjunction with kidney disease. Frostbite causes tissue death in feet but has clear environmental causation and typically bilateral, symmetrical presentation. Constriction injuries from fibers, bands, or leg bands create localized necrosis with identifiable external cause. Infectious diseases including bacterial, fungal, or viral infections can cause foot pathology with distinct appearance and treatment requirements. Accurate diagnosis ensures appropriate treatment selection.

Potential complications of foot necrosis syndrome can worsen prognosis and complicate management. Septicemia, or bloodstream infection, occurs when bacteria from necrotic tissue enter the circulation, potentially causing life-threatening systemic illness. Osteomyelitis develops when infection spreads from soft tissue to underlying bone, significantly complicating treatment. Progressive necrosis spreading beyond initial areas may necessitate more extensive surgery than originally anticipated. Chronic pain syndrome can persist even after tissue healing, requiring ongoing pain management. Psychological effects including depression and behavioral changes may develop in response to chronic illness and decreased mobility. Secondary injuries from falls related to foot dysfunction can create additional problems requiring treatment. Recognizing and addressing complications promptly improves outcomes for affected birds.
