Performance changes sometimes prompt radiographic investigation. A horse that's previously sound but is now struggling with specific movements, jumping awkwardly, or showing behavioral changes suggesting pain might need radiographs to identify bone or joint problems contributing to the issue.
Swelling of joints specifically points toward the need for radiographs. While ultrasound is excellent for assessing soft tissue swelling, radiographs are important for evaluating whether bone involvement is present. Joint radiographs help determine whether arthritis is developing and how advanced any existing arthritis is.
Recurrent lameness in the same location suggests chronic bone or joint issues that need radiographic evaluation. A horse that goes lame, recovers after rest, then goes lame again in the same area likely has underlying bone pathology that radiographs can identify.
Chewing problems or feed dropping from the mouth prompts dental and skull radiographs. Difficulty eating, excessive salivation, or foul-smelling breath might reflect dental disease that shows on radiographs. Facial swelling or asymmetry might indicate sinus disease visible on radiographs.
Respiratory or coughing issues sometimes warrant chest radiographs, though these require more sophisticated equipment and aren't as commonly taken. Radiographs of the cervical spine are appropriate when neurological signs suggest possible spinal involvement.
History of previous injury in an area sometimes prompts radiographs later if performance changes or new lameness develops. Evidence of previous bone damage or healing might explain current problems.
Section 4 Diagnosis And Treatment
Your veterinarian will discuss the need for radiographs based on clinical findings. The location of suspected lameness, the region you want to evaluate, and the information needed determine which radiographs are recommended. Your vet might recommend "a set of lower leg radiographs" or "radiographs of the foot" or "radiographs of both front legs for comparison."
Before radiographs are taken, your horse typically needs some preparation. The area being radiographed is cleaned to remove mud and dirt that might obscure detail. Sometimes the hair is clipped so radiographic plates or digital sensors can be placed close to the skin. For foot radiographs, the horse's hooves are usually cleaned thoroughly.

Most horses tolerate radiography well, particularly if they're accustomed to handling. Some horses need minimal sedation to stand patiently for the procedure, though most can be managed with just good handling and patience.
During the procedure, your veterinarian positions the radiographic equipment and takes images from multiple angles and positions. Different views allow visualization of structures from different perspectives. For example, foot radiographs might include side view, front-to-back view, and angled views to assess specific structures.
Once images are captured, they're reviewed immediately if digital radiography is available. Your veterinarian can often show you the radiographs and explain what's visible while you're still at the barn. Film radiographs require development and might need to be sent to a specialist for interpretation.
Interpretation of radiographs involves identifying any abnormalities and determining their significance. Sometimes radiographs show findings that explain the lameness. Sometimes findings are present but need ultrasound or other diagnostics for full assessment.

Your veterinarian might send radiographs to a board-certified radiologist or orthopedic specialist if the diagnosis isn't clear or if complex treatment decisions are being made. After radiographs are taken, your veterinarian explains the findings and what they mean for your horse. If radiographs are normal, you might be discussing why the horse is lame if lameness is being investigated—soft tissue involvement is likely. If radiographs show problems, your veterinarian discusses what's seen and what treatment options are available.
Understanding radiographic findings helps you make informed decisions. Ask your veterinarian to explain what problems are visible, whether they're acute injuries or chronic changes, and what the expected progression is. Radiographic findings don't always correlate with clinical signs. Some horses have significant radiographic changes but minimal lameness and continue performing well. Others have subtle radiographic findings but severe lameness. Your veterinarian interprets radiographic findings in context with clinical examination and the horse's actual performance.
Progression of radiographic findings is monitored over time with repeat radiographs. Some conditions stabilize after initial development. Others progress steadily. The rate of progression helps your veterinarian counsel you about expected timeline and future management.
Management decisions are based partly on radiographic findings. Horses with diagnosed fractures usually require specific treatment and rest. Horses with arthritis benefit from management strategies like medication, appropriate exercise, and potentially supplements or injections. Horses with radiographic findings that aren't causing current problems might be monitored without intervention.
Long-term outlook depends on what radiographs show and how seriously it affects the horse. A horse recovers from a healed fracture and returns to sound performance. A horse develops arthritis and might have limited high-level athletic career but can often be managed for light riding.
