A tendon injury is one of the scariest things that can happen to a working horse. I'm going to be direct about this because you need to understand the reality: tendon injuries are serious, recovery is long, and complete healing back to pre-injury function doesn't always happen. Accepting that reality early helps you make better decisions about your horse's future and respond appropriately to what happens.

Tendons are what connect muscle to bone. They transmit the force your horse generates with their muscles into movement. When a tendon tears—which can range from microscopic fibers to complete rupture—the damage interrupts that chain. The horse loses function, experiences pain, and faces a rehabilitation process measured in months, not weeks.

The most common tendon injuries are in the lower leg, particularly the flexor tendons running down the back of the cannon bone. These tendons are under tremendous stress during work, especially at speed. That stress, combined with uneven ground, fatigue, poor footing, or a misstep, can overwhelm the tendon's structural integrity and cause injury. Other tendons can be injured too, but the flexor tendons are most commonly affected in sport horses.

Severity varies. Some horses experience a mild strain—inflammation and microtearing but no complete rupture. Others suffer partial tears where some of the tendon fibers are completely torn but not the whole structure. Some experience complete rupture where the tendon is entirely severed. The severity determines recovery timeline and realistic expectations about return to function.

The horse's future after tendon injury depends on severity, location, quality of care during recovery, management decisions you make, and honestly, some luck with how healing happens. A mild strain with appropriate management might recover to near-normal function. A complete rupture in a competitive horse often means retirement from athletic work. Many injuries land somewhere in between, with partial recovery possible but not full return to pre-injury capability.

This is hard information to accept when you're dealing with an injured horse, but facing it straight allows you to make good decisions about what's reasonable to pursue for your individual horse's situation.

Section 2 Causes And Risk Factors

Tendon injuries happen when stress exceeds the tendon's structural capacity. A horse running on uneven ground, landing wrong, or pushing beyond fatigue can overwhelm a tendon. A sudden stop, a hard turn, or a slip can cause tearing. The horse doesn't have to do something dramatic—sometimes fatigue and accumulated micro-damage cause failure during routine work.

Work intensity and duration increase injury risk. A horse pushed hard before conditioning is adequate is more vulnerable to injury than a properly conditioned horse doing similar work. Young horses whose tendons aren't fully mature are more vulnerable than older horses. Conversely, very old horses sometimes have age-related degeneration that weakens tendons. The sweet spot for tendon strength is healthy adult horses doing work appropriate to their conditioning level.

Footing is critical. Uneven, slippery, or deep footing puts more stress on tendons. Working on hard ground, especially at speed, loads tendons heavily. Good footing distributes forces more evenly. Poor footing concentrates stress and increases injury risk. A horse working regularly on ideal footing has lower tendon injury risk than one working on challenging terrain.

Conformation affects vulnerability. Horses with inherent conformational weaknesses—angular limbs, toed-out or toed-in hooves, upright or overly sloped patterns—put different stresses on tendons. Balanced, clean limb conformation distributes forces more evenly. Horses with less-ideal conformation might be predisposed to injury. This doesn't mean they can't be ridden safely, but they might have different load limitations.

Previous injury significantly increases reinjury risk. A horse that once injured a tendon has weakened tissue, scar formation, and residual structural change. That tendon is more vulnerable to future injury than it would have been without damage. Many horses that injure a tendon once injure it again or develop problems in the opposite leg as compensation. This is why tendon injuries are chronic concern, not one-time problems.

Fatigue increases risk. A tired horse doesn't have the muscular support and coordination that prevents injury. This is why tendon injuries happen late in rides, late in seasons, or in horses pushed beyond their conditioning. A fresh, well-conditioned horse using their muscles efficiently has better tendon support than a tired, unconditioned horse.

Temperature affects tissue quality. Cold tendons are more brittle and susceptible to injury. This is why tendon injuries happen more frequently in cold weather. Warm-up before work helps prepare tendons for stress. Allowing adequate warm-up time before intense effort reduces injury risk. Conditioning and consistent work maintains tendon flexibility and strength. Inconsistent work, long time off, and jumping into activity increases vulnerability.

Section 3 Signs And Symptoms

The acute signs of tendon injury are dramatic. A horse suddenly goes lame, often severely. The affected leg might swell dramatically—you see obvious puffing in the area of the tendon. The horse might be barely weight-bearing and showing signs of pain. Some horses are more stoic, but the lameness is clear. Swelling develops over hours, becoming most obvious 12 to 24 hours after injury.

Heat and pain accompany the swelling. The injured area feels hot to the touch and is very sensitive to palpation. The horse reacts to pressure on the tendon. Flexing and extending the leg reproduces pain. You might see the horse standing with the affected leg in partial flexion, not extending it fully because extension is painful.

In the immediate aftermath, the horse might refuse to put weight on the leg at all. Some horses have so much pain they're three-legged lame. Others with less severe injuries might hobble along with minimal weight-bearing. The severity of initial lameness doesn't always correlate perfectly to severity of injury, but severe lameness with significant swelling indicates significant injury.

The horse's behavior reflects pain. They might be anxious, sweating from pain, or seeming distressed. Some horses are stoic and show less behavioral response despite significant injury. But the physical signs—lameness, swelling, heat, pain on palpation—are unmistakable indicators something serious has happened.

In the days following acute injury, swelling might increase before it starts decreasing. Heat might persist for weeks. Lameness improves slightly as acute inflammation peaks and starts subsiding, but the horse remains lame for extended periods. Initial improvement in lameness is often rapid but then plateaus, indicating that structural healing is the limiting factor, not just inflammation.

Subacute and chronic signs develop over weeks and months. The horse remains lame though improving gradually. Lameness might disappear at walk but become apparent at trot or canter. The horse might move well in some directions but not others. Swelling might resolve, leaving a slightly thickened appearance to the tendon. The tendons become visible—you can often see where the injury was because that tendon doesn't look quite right compared to the opposite leg.

Complicating factors emerge during recovery. Some horses develop secondary injuries—scar tissue limitations, compensatory injuries to the other leg, or problems in other joints. Some develop chronic inflammation or drainage if infection occurred. The path isn't always straightforward forward progress—sometimes there are setbacks.

Section 4 Diagnosis And Treatment

Your vet diagnoses tendon injury through physical examination combined with ultrasound imaging. The physical exam reveals lameness, swelling, heat, and pain on palpation in the tendon area. Ultrasound shows the actual structure of the tendon—whether there are tears, how extensive they are, and whether there's inflammation or fluid. Ultrasound is the definitive diagnostic tool because it shows exactly what's damaged.

Some vets use radiographs to rule out bone involvement or fracture. While tendons don't show well on X-rays, bone damage can occur alongside tendon injury. MRI can provide more detailed information but is expensive and not always necessary. Ultrasound gives most of the information your vet needs to make treatment recommendations.

Treatment for tendon injury focuses on supporting healing and managing pain. The immediate goal is rest and controlled movement to allow damaged tissue to begin healing without further injury. Complete stall rest for the first one to two weeks decreases inflammation and allows the initial healing phase. Anti-inflammatory medication—NSAIDs like phenylbutazone—reduces swelling and pain.

After the acute phase, controlled hand-walking begins. Short periods of hand-walking at a walk, increasing gradually, stimulates healing without overstressing the tendon. The horse might progress from five minutes to ten minutes to twenty minutes of hand-walking over weeks. Trotting or cantering is not resumed for extended periods—we're talking weeks to months of walking only. This seems slow but allows proper healing.

Managing swelling and supporting the limb helps. Ice or cold water therapy in the first few days reduces inflammation. Compression bandaging supports the tendon. Some facilities use other modalities—ultrasound therapy, shock wave therapy, or stem cell therapy—with varying evidence of benefit. Your vet advises what's appropriate for your horse's situation.

Medication management includes NSAIDs during the painful phase, but long-term NSAID use has complications, so it's typically tapered as pain improves. Some horses benefit from other analgesics or anti-inflammatory approaches. Injectable medications into the tendon sheath or joint—if joint involvement exists—might be used. Your vet determines appropriate medications.

Surgical intervention is considered for complete ruptures or injuries severe enough that non-surgical management is unlikely to succeed. Tendon repair surgery has limited success in horses, so it's reserved for very specific situations. Most tendon injuries are managed conservatively with rest and controlled rehabilitation.

Returning to work is gradual and cautious. It might take months of progressive movement before the horse is back to light work. Even then, some horses never return to previous levels of competition. The timeline depends on severity—a mild strain might return to work in three to six months, a moderate injury might take nine to twelve months, a severe injury might take over a year or might never fully recover.

Section 5 Management And Care

During acute recovery, management focuses on minimizing stress and supporting healing. Stall rest for the first phase requires careful monitoring to prevent pressure sores and maintain the horse's mental health. Some facilities allow controlled hand-walking as part of early management. Feeding should support healing—appropriate calories to maintain weight, adequate protein for tissue repair, and micronutrients that support healing.

Compression bandaging needs changing regularly and must be done properly to avoid pressure points. Cold therapy in the first few days helps, though benefit past the acute phase is debated. Many horses benefit from standing in ice water or having ice boots applied. Supporting the limb reduces stress on the healing tendon.

As the acute phase passes and controlled rehabilitation begins, management becomes more complex. Hand-walking must be consistent—same time daily, same duration that gradually increases. The handler needs to understand the program well enough to execute it correctly. A horse walked intensely one day then rested several days isn't progressing properly. Consistency is critical.

Monitoring progress involves regular veterinary ultrasound exams. Initial ultrasound establishes the baseline damage. Follow-up ultrasounds weeks later show healing progression. Some vets do ultrasound monthly to track healing and adjust the rehabilitation program. Without ultrasound confirmation, you're guessing about whether healing is progressing appropriately.

Pain management during recovery maintains the horse's quality of life. Chronic pain from healing tendon damage is difficult. Some horses are more lame weeks into recovery than they were initially, suggesting complications or excessive rehabilitation stress. If lameness worsens rather than improving, that's information that the rehabilitation program needs adjustment.

Nutrition that supports healing becomes important. Adequate protein, minerals like copper and zinc, and calories for tissue repair matter. Some horses benefit from supplements specifically formulated to support tendon healing, though evidence is mixed. The foundation is good quality hay and grain, then you can consider supplementation based on your vet's recommendations.

Planning for the horse's future is part of managing a tendon injury. Some owners face the difficult decision of whether trying to rehabilitate a severely injured horse is realistic or whether retirement or euthanasia is kinder. Moderate injuries might return to limited work or riding at walk only. Severe injuries often end athletic careers. Understanding your horse's injury severity helps you make realistic decisions about what to pursue.

Section 6 Prevention And Outlook

Preventing tendon injuries involves conditioning appropriately, managing footing and environment, and respecting your horse's limitations. A horse conditioned gradually for the work expected of them has better tendon strength and coordination to prevent injury. Proper warm-up before intense work prepares tendons and muscles. Cool-down afterward prevents overloading of tired tendons. These basic practices reduce injury risk significantly.

Footing matters tremendously. Working on good footing, avoiding uneven terrain when possible, and being especially careful on slippery or hard ground reduces injury risk. Farrier work that maintains balance and healthy feet supports tendon health. Hoof problems change loading patterns and can increase tendon stress.

Recognizing individual horse limitations helps prevent injury. Some horses are naturally fragile or have conformation that predisposes to injury. Working these horses within their limits—maybe avoiding intense speed work, avoiding difficult terrain, keeping work moderate—prevents injury. Not every horse is meant for competition or difficult work. Accepting that helps prevent injury from pushing a horse beyond their capability.

Tire management is real. A tired horse is injury-prone. Stopping before exhaustion, avoiding work when a horse is already fatigued, and respecting individual fitness levels prevents many injuries. Racing or jumping a tired horse is asking for injury.

The prognosis for tendon injury depends entirely on severity and your commitment to proper rehabilitation. Mild strains often recover to full function. Moderate injuries might recover 80-90 percent of pre-injury function. Severe injuries might recover 50-60 percent or might not return to athletic work at all. Complete ruptures often end athletic careers. These aren't guarantees—individual healing varies—but they're realistic expectations.

Long-term outlook after tendon injury requires accepting that the horse might never be quite the same. Even successfully healed tendons are vulnerable to reinjury. A horse that healed from one tendon injury needs careful management to prevent another. This might mean retirement from competitive work, might mean limiting work to trail riding, might mean managing pain longterm. It's not necessarily the end of the horse's useful life, but it changes what's possible.

The hard reality is that some tendon injuries are career-ending. A young performance horse with a severe tendon injury faces uncertain future for competition. An older horse with less demanding work might recover acceptably. A horse with multiple injuries or chronic issues might reach a point where quality of life becomes the primary concern.

Preventing the injury is infinitely better than dealing with recovery. Understanding that tendon injuries are serious, taking prevention seriously through conditioning and management, and respecting your horse's limitations keeps many horses sound throughout their careers.
