The key is recognizing when you have a disease problem, isolating the sick horses immediately, and implementing basic sanitation and management to prevent it from spreading to everyone else.
Contagious diseases in horses range from minor respiratory viruses that most horses recover from with rest and time, to serious conditions like strangles or equine infectious anemia that are reportable to authorities and require specific protocols. Some diseases spread through direct contact. Some spread through respiratory droplets. Some spread through contaminated feed, water, or equipment. Some are spread by insects or contaminated hands and clothing.
The difference between a single sick horse and an outbreak is isolation. A single sick horse that gets separated immediately might infect nobody. The same horse left to mix with others can get your entire facility sick in days. That's the power of early action.
Your response to a sick horse is different if you have one horse versus a facility with many. If you have one horse, you get your vet to figure out what's going on and you treat it. If you have a facility, you need protocols for containment because you're protecting everybody.

Section 2 Causes And Risk Factors
Viral infections are probably the most common cause of disease outbreaks in horse facilities. Equine infectious anemia is a virus spread through blood-to-blood contact. It's serious and there's no cure, just management. Horses test positive for life and can't be moved between facilities in many states.
Parasites spread through fecal contamination of pastures and water. Heavy parasite burden in a facility can sicken multiple horses and be hard to eradicate.
Respiratory syncytial virus in foals and young horses spreads through respiratory droplets and causes severe respiratory disease.
Cryptolepis, Potomac horse fever, and other infectious diseases are geographic or seasonal. Your location and time of year affect what diseases might show up.
Risk factors for outbreaks include overcrowding, poor ventilation in barns, high-stress situations that compromise immune systems, poor sanitation, shared equipment and water sources, and lack of quarantine protocols when new horses arrive.
Animals purchased from unknown sources or showing unclear health history are higher risk for bringing in disease. Horses that come from auctions or mixed facilities might be carrying disease before showing symptoms.
Nutrition and stress affect immune response. Horses that are malnourished or chronically stressed get sick more easily and can't fight off infection as well.
Age matters. Very young foals have immature immune systems and can get severely ill from diseases adult horses handle. Very old horses with compromised immunity are also vulnerable.

Section 3 Signs And Symptoms
Fever is often the first sign that something's contagious. A temperature above 101 degrees in a horse that was normal yesterday is a red flag. If multiple horses develop fever around the same time, you're probably dealing with something spreading.
Respiratory signs like cough, nasal discharge, and labored breathing suggest a respiratory disease outbreak. One horse coughing might be nothing. Multiple horses coughing in a facility within days of each other is a pattern that suggests contagious disease.
Lethargy and depression show you horses feel genuinely sick. A horse that was normal a few days ago and now is standing quietly and not interested in food is showing you they're fighting off infection.
Swollen lymph nodes, especially under the jaw and along the neck, suggest strangles or other bacterial infection. These might progress to draining abscesses if the disease is strangles.
Nasal discharge that's yellow or thick suggests bacterial infection. Clear discharge might be viral. Any discharge combined with fever and cough in multiple horses is concerning.
Reduced appetite or difficulty swallowing happens with diseases that affect the throat or mouth. Horses having trouble eating or swallowing while also running fever is a red flag.
Abdominal signs like diarrhea or colic in multiple horses suggest something systemic is affecting the herd.
Neurological signs like incoordination, paralysis, or behavior changes are serious and suggest a neurological disease spread through the facility. This is an emergency.
Abortions in pregnant mares, especially if multiple mares abort around the same time, suggest a disease like equine herpesvirus affecting the herd.
Mild signs that progress over days in multiple horses build a pattern that something contagious is happening. Early signs in one horse that later appear in others confirm transmission.
Section 4 Diagnosis And Treatment
Your vet will take history on how the disease started, how many horses are affected, what signs they're showing, and how rapidly it's spreading. This tells them a lot about what they're dealing with.
Temperature taking in multiple horses helps establish if there's a pattern of fever across the herd.
Viralculture or PCR testing confirms what virus or bacteria is causing the disease. Your vet collects nasal swabs, blood samples, or other samples depending on what they suspect and sends them for testing. Results tell you exactly what you're dealing with and what protocols apply.
Some diseases are reportable to state or federal animal health officials. Your vet knows what falls in that category and will report if required. Strangles and equine infectious anemia are reportable in most states.
Treatment depends on the disease. Viral diseases are mostly supportive treatment like rest, fluids, and pain medication if needed. Antibiotics don't help viral infections. Some viruses warrant specific antivirals if caught early.
Bacterial infections like strangles need antibiotics. The challenge is that early treatment might prevent abscess formation which helps the immune system, so timing matters. Your vet knows when and how to treat.
Equine infectious anemia has no cure. Positive horses require quarantine and management to prevent transmission.
Management of an outbreak at a facility requires isolation of sick horses, quarantine protocols for exposed horses, and cleaning and disinfection of contaminated areas.
Vaccination of unaffected horses might be recommended depending on the disease and timing. Some vaccines work as prevention, others don't help once exposure has occurred.
Prognosis varies by disease. Many viral diseases most horses recover from with supportive care. Strangles has good survival rates with proper treatment. Serious diseases like West Nile virus or equine infectious anemia have variable outcomes depending on how severely the horse is affected.
Some horses recover completely. Others have lasting effects from severe illness. Some don't survive, which is why prevention and early treatment matter.
Section 5 Management And Care
Isolation is the most important management tool when horses get sick. Move sick horses to a separate area away from unaffected horses. They should not share pastures, hay, water, or equipment. This means separate stalls, separate water buckets, separate feed containers. Completely separate if possible.
Quarantine protocols mean anyone handling sick horses should not go directly to handle well horses. Change clothes and wash hands between handling sick and healthy animals. If that's not practical because of facility layout, handle well horses first then sick horses, not the other way around.
Dedicated equipment for sick horses prevents transmission. If possible, have separate brushes, halters, saddles, and tack for the quarantine area. Don't move equipment back and forth.
Cleaning and disinfection of contaminated areas is part of outbreak management. Barn areas, water troughs, and gates that sick horses use should be cleaned and disinfected. Different disinfectants work on different organisms so your vet might recommend specific products.
Water management means clean fresh water for sick horses and separate water sources for well horses if possible. Contaminated water troughs spread disease rapidly.
Feed management means separate feed for sick horses that's not shared with well horses. No hay contaminated by sick horses going to well horses.
Ventilation is important. If sick horses are in a barn, make sure air is moving and the space isn't stagnant. Virus particles settle in still air.
Observation of well horses is essential during an outbreak. Check temperature twice daily on horses that were exposed. Watch for any early signs of illness. Early recognition of new cases means rapid isolation and containment.
Medicinal treatment as prescribed by your vet is important. If horses need antibiotics, give them as directed. If they need supportive care with fluids or other treatment, make sure that happens.
Rest for sick horses means minimal exercise or handling. A horse fighting infection needs energy to do that, not energy spent being ridden or worked.
Nutrition support means making sure sick horses eat despite feeling bad. Soft feed, hay soaked in water, frequent small meals can help. Some horses won't eat much while sick but will drink, so water access is critical.
Return to normal housing and mixing should be gradual and based on recovery. A horse that seems better might still be shedding virus and spreading disease. Your vet advises when it's safe to return to normal.
Section 6 Prevention And Outlook

Some horses might never return to full function after serious illness. Neurological damage, permanent respiratory effects, or other complications can linger. But with good management, most horses recover and go back to normal.
Facility recovery after an outbreak involves thorough cleaning and disinfection before bringing in new horses. Some facilities have to depopulate completely and start fresh to eradicate certain diseases.
Long-term, a well-managed facility with good biosecurity, vaccination protocols, and quarantine procedures prevents most disease outbreaks before they start. When problems do happen, quick response prevents them from becoming catastrophic.
