Behavioral changes often accompany bloat when it results from illness rather than simple overeating. The fish may become less active than usual, spending more time resting on the bottom or hovering in one spot. Appetite typically decreases as discomfort increases, with severely bloated fish refusing food entirely. Buoyancy problems sometimes develop alongside bloat, especially when air accumulation or swim bladder issues are involved. When bloat progresses to the point where scales begin to stick outward from the body, creating a pinecone-like appearance when viewed from above, the fish has developed dropsy. This indicates severe fluid accumulation and usually means organs are failing. Dropsy is technically a symptom rather than a disease itself, but the underlying causes have progressed to the point where recovery is unlikely. Fish showing dropsy often have bulging eyes, extremely labored breathing, and complete loss of appetite.
Feces characteristics provide clues about digestive bloat. Constipated fish may trail long strings of feces that hang from the vent without detaching. The feces may be unusually pale, thin, or mucousy compared to healthy droppings. Some bloated fish produce no visible feces at all because material is backed up in the intestines. Normal, well-formed feces that break off cleanly suggest the digestive system is functioning and bloat may have another cause.
Section 4 Treatment Options
The first response to bloat in most cases should be fasting. Stop feeding the fish for two to three days to allow the digestive system to clear and any accumulated food to pass through. Epsom salt also acts as a muscle relaxant that may help constipated fish pass blockages. This treatment can be repeated daily for several days if the fish tolerates it well.
For bloat suspected to result from bacterial infection, antibiotic treatment may be necessary. Medications containing kanamycin or metronidazole are commonly used for internal bacterial infections. Metronidazole is particularly indicated for Malawi bloat and other conditions affecting cichlids. Treatment should begin promptly once bacterial infection is suspected because internal infections progress quickly. Medicated foods deliver medication directly to the gut if the fish is still eating.
Adjust the environment during treatment to reduce stress and support recovery. Raise temperature slightly if appropriate for the species, as warmth speeds metabolism and may help the fish process whatever is causing the bloat. Ensure excellent water quality with extra attention to ammonia and nitrite levels. Dim lighting and minimal disturbance help the fish rest and direct energy toward healing.
If the fish has developed full dropsy with raised scales, treatment is unlikely to succeed but can be attempted if you wish to give the fish every chance. Aggressive antibiotic treatment combined with Epsom salt baths sometimes reverses early dropsy, but the survival rate is low. Many fishkeepers choose humane euthanasia at this stage rather than prolonging suffering with treatments that rarely work.
Prevention of recurrence requires addressing whatever caused the bloat initially. If overfeeding was the issue, reduce portion sizes and feeding frequency. If diet composition caused the problem, research appropriate foods for your species and adjust accordingly. If internal infection was responsible, investigate what stressed the fish enough to let infection take hold and correct those conditions.

During treatment for bloat, maintain the tank with extra attention to cleanliness and stability. A fish dealing with digestive or internal problems does not need the additional stress of fluctuating parameters or accumulating waste. Test water regularly and perform water changes as needed to keep conditions optimal. Siphon the substrate to remove any uneaten food or waste that might decompose and affect water quality.
Consider moving the bloated fish to a hospital tank for treatment, especially if you need to modify water conditions or add medications that could affect tankmates. A bare-bottom tank with a heater and gentle filtration makes observation easy and allows you to adjust salt concentrations or medications precisely. Hospital tanks also protect other fish from exposure to any pathogens if infection is causing the bloat.
Adjust feeding for the entire tank if multiple fish show signs of bloat or if you suspect overfeeding contributed to the problem. Skip a day or two of feeding for all inhabitants, then resume with smaller portions. Review how much you have been feeding compared to guidelines for your fish species.
