Inner ear infection, known medically as otitis interna, is by far the most common cause of vestibular disease in guinea pigs. These infections typically begin as middle ear infections that spread deeper into the bony structures housing the vestibular apparatus. The bacteria most commonly involved include Bordetella bronchiseptica, Streptococcus pneumoniae, and Pasteurella species, many of the same organisms responsible for upper respiratory infections. In fact, a URI that is inadequately treated or that progresses despite treatment can extend through the Eustachian tube into the middle ear and eventually the inner ear, making untreated respiratory infections a significant risk factor for vestibular disease.
Middle ear infections, called otitis media, are the precursor to inner ear involvement in most cases. These infections can develop from bacteria ascending through the Eustachian tube from the throat, from extension of an outer ear infection traveling inward, or from bacteria reaching the ear through the bloodstream during a systemic infection. A guinea pig with a middle ear infection may show subtle signs like slight head tilting, ear sensitivity, or changes in appetite before the more dramatic vestibular symptoms appear. Unfortunately, middle ear infections in guinea pigs are notoriously difficult to detect externally because the infection is deep within the skull and the eardrum may appear normal on external examination.
Encephalitozoon cuniculi is a microscopic parasitic organism that can infect guinea pigs and cause inflammation in the brain and kidneys. When it affects the brain regions responsible for balance processing, vestibular symptoms result. This parasite is more commonly associated with rabbits but has been documented in guinea pigs as well. Diagnosis can be challenging because blood tests detect antibody exposure rather than active infection, and treatment involves extended courses of antiparasitic medication.
Trauma to the head or ear can damage the vestibular structures and produce sudden onset vestibular symptoms. Falls from height, being dropped during handling, or being stepped on can all cause enough impact to disrupt the delicate inner ear structures. Traumatic vestibular disease typically has a very sudden onset immediately following the injury, which helps differentiate it from the more gradual onset associated with infection.
Brain lesions, including abscesses, tumors, and areas of inflammation, can cause vestibular symptoms when they affect the parts of the brain that process balance information. These are referred to as central vestibular disease, in contrast to the peripheral vestibular disease caused by inner ear problems. Toxin exposure and certain medications can temporarily or permanently damage the vestibular apparatus. Age-related vestibular degeneration, sometimes called idiopathic vestibular disease, can occur in older guinea pigs without an identifiable underlying cause. Section 3 Symptoms
The symptoms of vestibular disease in guinea pigs are among the most visually striking of any condition and are usually impossible to miss once they develop. The onset can be gradual over several days or strikingly sudden, depending on the underlying cause.
Head tilt is the signature symptom of vestibular disease. The guinea pig holds their head at an angle, with one ear consistently lower than the other. The direction of the tilt usually corresponds to the side of the affected ear or brain region. The tilt may be mild, with just a slight angle, or severe, with the head turned nearly sideways. In early or mild cases, the tilt may come and go, but as the condition progresses it typically becomes constant.
Loss of balance and coordination, known as ataxia, accompanies the head tilt. The guinea pig may stagger, weave, and stumble when trying to walk, leaning heavily toward the affected side. They may fall over repeatedly when attempting to move. In severe cases, the guinea pig may be unable to stand at all and may roll continuously toward the affected side, a terrifying symptom called rolling or barrel rolling. Guinea pigs experiencing rolling episodes can injure themselves on cage walls and objects if not protected.
Circling behavior is common, where the guinea pig walks in tight circles toward the affected side. This happens because the disrupted vestibular signals cause the brain to perceive constant rotation, and the guinea pig moves in response to that perceived movement. The circling may be intermittent in mild cases or nearly constant in severe ones.
Nystagmus is an involuntary rapid movement of the eyes, either side to side or in a rotary pattern. The eyes appear to flick or bounce rhythmically, and the guinea pig cannot control this movement. If the underlying cause is an ear infection, additional symptoms related to the infection may be present. These can include discharge from one or both ears, scratching or pawing at the ear, sensitivity or pain when the ear area is touched, and concurrent respiratory symptoms like nasal discharge and sneezing if the infection originated as a URI.
In cases of central vestibular disease involving the brain, additional neurological symptoms may be observed beyond the typical balance disruption. These can include weakness or paralysis on one side of the body, changes in consciousness or awareness, abnormal reflexes, and seizures. Central vestibular symptoms are generally more severe and less responsive to treatment than peripheral vestibular disease originating in the inner ear.
The severity of symptoms does not always correlate directly with the severity of the underlying condition. Some guinea pigs with relatively minor infections can show dramatic vestibular symptoms, while others with more serious underlying problems may show more gradual onset. Regardless of symptom severity, any guinea pig displaying vestibular signs needs veterinary evaluation as soon as possible.
Section 4 Diagnosis
Diagnosing vestibular disease in guinea pigs involves confirming the vestibular dysfunction and then working to identify the underlying cause, which determines the appropriate treatment approach. This evaluation should be performed by an exotic veterinarian with experience in guinea pig neurology and ear conditions.
The neurological examination is the starting point. The veterinarian assesses the guinea pig's balance, gait, head position, and eye movements. The direction of the head tilt, the direction and type of nystagmus, and the presence or absence of other neurological deficits help differentiate between peripheral vestibular disease originating in the inner ear and central vestibular disease originating in the brain. This distinction is clinically important because peripheral disease is more common and generally more treatable, while central disease often carries a more guarded prognosis.
Otoscopic examination allows the veterinarian to look into the ear canal and evaluate the eardrum. A ruptured or discolored eardrum suggests middle ear infection that may have extended to the inner ear. However, a normal-appearing eardrum does not rule out middle or inner ear infection, because infection can be present behind an intact eardrum. This limitation means that additional imaging is often necessary.
Radiographs of the skull can reveal changes in the tympanic bullae, the bony chambers that house the middle ear. Infection in the middle ear causes the walls of the bullae to thicken, and fluid or pus accumulation may be visible as increased opacity. Radiographs are useful as a screening tool but have limited sensitivity for detecting early or subtle changes.
CT scan is the gold standard imaging modality for evaluating the middle and inner ear structures in guinea pigs. CT provides detailed cross-sectional images that can reveal fluid accumulation, bone destruction, soft tissue masses, and other abnormalities within the tympanic bullae and surrounding structures with much greater clarity than radiographs. If available and financially feasible, CT imaging provides the most accurate assessment of the extent of ear involvement.
MRI may be recommended if central vestibular disease is suspected, as it provides superior imaging of the brain and can reveal abscesses, tumors, inflammatory lesions, and other abnormalities that CT does not visualize as well. MRI is less widely available for exotic animals and is more expensive than CT, but it provides essential information when brain involvement is suspected.
Blood work including a complete blood count and chemistry panel helps assess overall health, detect signs of systemic infection, and evaluate organ function. Testing for Encephalitozoon cuniculi antibodies may be performed if this parasite is suspected, though interpreting the results requires clinical judgment since a positive antibody test indicates exposure but does not confirm active disease.
Bacterial culture of ear discharge, if present, can identify the specific organism responsible for the infection and guide antibiotic selection. In cases where the eardrum is intact but middle ear infection is suspected, culture may be obtained during a myringotomy procedure, where the eardrum is deliberately punctured under anesthesia to access and drain the middle ear contents.
Section 5 Treatment
Treatment for vestibular disease targets the underlying cause while providing supportive care to manage the balance disruption and maintain the guinea pig's nutrition and comfort during recovery.
For bacterial inner ear infections, which are the most common cause, aggressive antibiotic therapy is the cornerstone of treatment. The antibiotics chosen must be effective against the likely organisms, safe for guinea pigs, and capable of penetrating the bony structures of the middle and inner ear where the infection resides. Baytril (enrofloxacin) is frequently used because it achieves good concentrations in bone tissue. Chloramphenicol is another option that penetrates well into the ear structures. In some cases, combination antibiotic therapy is prescribed to broaden coverage. Treatment courses for ear infections causing vestibular disease are typically long, often four to six weeks or longer, because the infection is deep-seated and the antibiotic must penetrate dense bone.
Anti-inflammatory medication helps reduce swelling and inflammation in the inner ear structures that is contributing to vestibular dysfunction. Meloxicam is commonly prescribed for its anti-inflammatory and pain-relieving properties. In some severe cases, a short course of corticosteroids may be considered to rapidly reduce inflammation, though this decision is weighed carefully because steroids can suppress the immune system.
Anti-nausea medication may be prescribed to manage the motion sickness-like symptoms that accompany vestibular disease. Reducing nausea encourages eating, which is critical for preventing GI stasis. Medications like meclizine or maropitant may be used depending on the veterinarian's preference and the guinea pig's response.
If Encephalitozoon cuniculi is suspected, treatment involves antiparasitic medication, typically fenbendazole, administered daily for a prolonged period of at least 28 days. The response to treatment can be slow, and some damage caused by the parasite may not be fully reversible, but treatment can halt progression and allow partial recovery.
For cases requiring surgical intervention, such as a middle ear abscess that needs to be drained, a bulla osteotomy may be performed. This procedure involves surgically opening the tympanic bulla to remove infected material and allow drainage. It is a specialized surgery that requires an experienced exotic surgeon but can be curative when infection is localized to the middle ear.
Supportive care is essential throughout the treatment period. Critical care syringe feeding maintains nutrition in guinea pigs that are too disoriented or nauseous to eat on their own. Fluid therapy addresses dehydration from reduced water intake. Keeping the guinea pig in a padded, confined space prevents injury from falling and rolling. The supportive care component of treatment is often what keeps the guinea pig alive and stable while the antibiotic or other targeted therapy has time to take effect.
Section 6 Homecare
Home care for a guinea pig with vestibular disease focuses on safety, nutrition, and patience. The recovery period can be lengthy, and your daily care during this time is essential for a positive outcome.
Create a safe recovery environment that prevents injury from the balance disruption. Line the cage with thick, soft padding such as folded fleece blankets to cushion falls. Remove all ramps, elevated platforms, water bottles mounted at height, and any hard objects that the guinea pig could collide with while stumbling or rolling. A smaller, enclosed recovery space is safer than a large cage during the acute phase. Some owners use a low-sided plastic storage container lined with fleece as a temporary recovery pen.

Vestibular symptoms often improve before the infection is fully cleared, and stopping treatment early risks relapse. Keep a medication log to track doses and times.
Monitor your guinea pig's progress by observing the severity of the head tilt, the frequency of falls or rolling episodes, appetite, water intake, activity level, and weight. Improvement is often gradual and may not be noticeable day to day but becomes clear when you compare week to week. Daily weigh-ins on a kitchen scale provide objective data about whether the guinea pig is maintaining adequate nutrition.
Be patient and do not expect overnight improvement. Recovery from vestibular disease, particularly when caused by inner ear infection, often takes weeks to months. The head tilt frequently improves significantly but may never resolve completely. Many guinea pigs retain a mild permanent head tilt after vestibular disease, and this is considered an acceptable outcome as long as the guinea pig has adapted well and maintains good quality of life. Guinea pigs are remarkably adaptable animals and most learn to compensate for a residual tilt, eating, drinking, moving, and interacting normally despite the slight angle.
Section 7 Prevention
Preventing vestibular disease focuses primarily on preventing the ear infections that are its most common cause, along with general health practices that support a strong immune system and reduce the risk of the less common causes.
Prompt and complete treatment of upper respiratory infections is one of the most important preventive measures. URIs that are not treated, treated inadequately, or treated with the wrong antibiotics can spread through the Eustachian tubes into the middle and inner ear. If your guinea pig develops a URI, seek treatment from an exotic veterinarian promptly, use the prescribed antibiotic for the full course, and attend all follow-up appointments to confirm the infection has cleared. A URI that appears to resolve but was not fully eliminated can smolder and eventually extend to the ear.
Environmental management that reduces respiratory infection risk also reduces vestibular disease risk. Maintain clean, well-ventilated housing with dust-free bedding. Avoid drafts and temperature extremes. Keep ammonia levels low through regular cage cleaning. These measures protect the respiratory tract and by extension reduce the chance of bacteria reaching the ears.
Proper nutrition supports the immune system that fights off the infections underlying most vestibular disease. Adequate daily vitamin C through fresh vegetables like bell peppers is essential because guinea pigs cannot produce their own vitamin C and deficiency directly impairs immune function. A balanced diet of timothy hay, timothy-based pellets, and varied fresh vegetables provides the nutritional foundation for a guinea pig that can effectively fight off bacterial challenges.
Separate guinea pigs from rabbits, which are common carriers of Bordetella bronchiseptica. This bacterium is responsible for many respiratory and ear infections in guinea pigs. Rabbits can carry it asymptomatically and transmit it through shared airspace. Housing guinea pigs and rabbits apart eliminates this transmission route.

Early detection and treatment of a middle ear infection is far simpler and more effective than treating full-blown vestibular disease after the infection has reached the inner ear.

Guinea pigs whose inner ear infections are treated with a full, extended course of antibiotics, typically four to six weeks, have better outcomes than those whose treatment is cut short. The deep, bony location of the infection means that antibiotics need prolonged time to fully penetrate and sterilize the affected area. Incomplete treatment leads to relapse and potentially to a more resistant infection that is harder to treat the second time.
Vestibular disease caused by Encephalitozoon cuniculi has a variable prognosis. Some guinea pigs respond well to antiparasitic treatment and show significant improvement, while others have limited recovery, particularly if the parasite has caused significant brain inflammation. Early treatment generally produces better results than delayed treatment.
Traumatic vestibular disease has a prognosis that depends entirely on the severity of the injury.
