A guinea pig that was fine this morning can be in critical condition by tonight if a stone shifts and blocks the urethra. There is no safe period of watchful waiting with a suspected urinary blockage. Ultrasound can detect stones that may not be clearly visible on X-rays, assess whether the kidneys show signs of backup pressure from the blockage, known as hydronephrosis, and evaluate the thickness and condition of the bladder wall. Hydronephrosis is a serious finding that indicates the blockage has been present long enough for pressure to back up into the kidneys, which worsens the prognosis.

Blood work may be performed to assess the guinea pig's metabolic status, particularly if the blockage appears to have been present for some time. Key values include blood urea nitrogen and creatinine, which indicate kidney function. Elevated levels of these waste products confirm that the kidneys have been unable to clear them effectively, a condition called azotemia. Electrolyte levels, particularly potassium, are also checked because dangerous potassium elevation can result from the inability to excrete this mineral, and elevated potassium can cause fatal heart rhythm abnormalities.

A urinalysis may be performed if a urine sample can be obtained, either from a small amount that the guinea pig manages to pass or via cystocentesis. The urinalysis can reveal the presence of blood, infection, crystals, and other abnormalities that inform both the immediate treatment plan and longer-term management strategies.

The urgency of the diagnostic process cannot be overstated. An experienced exotic veterinarian will move through these diagnostic steps efficiently, often performing the physical exam and radiographs within the first few minutes of the visit. Section 5 Treatment

Treatment for a urinary blockage in guinea pigs is always urgent and the specific approach depends on the location and nature of the obstruction, the guinea pig's overall condition, and whether complications have already developed. Fluid therapy is a critical component of treatment for any guinea pig with a urinary blockage. Intravenous or subcutaneous fluids are administered to correct dehydration, flush toxins from the bloodstream, support kidney recovery, and restore electrolyte balance. Fluids may need to be continued for several days depending on how compromised the kidneys were by the time of treatment.

Offering a water dish alongside the bottle gives guinea pigs an alternative drinking method. Some guinea pigs drink significantly more from dishes than bottles, and the increased intake can make a real difference in urinary health.

Regular veterinary screening is particularly important for guinea pigs with any history of urinary issues. Periodic radiographs can detect stones while they are still small enough to monitor or manage conservatively, before they become large enough to cause a blockage. Urinalysis can reveal elevated calcium levels, crystal formation, and early signs of infection. Maintaining a healthy weight through proper diet and adequate exercise space helps prevent the sedentary behavior that contributes to concentrated urine and mineral settling in the bladder. Environmental cleanliness reduces the risk of urinary tract infections that can serve as a starting point for stone formation. Clean bedding, dry living conditions, and regular cage maintenance all contribute to a lower bacterial load in the environment and around the guinea pig's urinary opening.

Knowing the early warning signs of urinary trouble and acting on them promptly is itself a form of prevention against blockage. Squeaking during urination, blood in the urine, frequent straining, and changes in urine consistency are all signals that should trigger a veterinary visit. Addressing these problems before they progress to a complete blockage is far safer, less expensive, and less stressful for everyone involved.

Most guinea pigs begin eating and moving around within 24 to 48 hours after surgery, and full recovery from the surgical incision takes approximately ten to fourteen days. The immediate postoperative period carries the highest risk, and close monitoring during this time is essential to catch any complications such as wound infection, return of obstruction, or surgical site dehiscence.

The long-term prognosis after a blockage is more complicated because of the high risk of recurrence. The underlying metabolic tendency that caused the original stone to form does not go away after surgery. Studies and clinical experience show that a significant percentage of guinea pigs that have had bladder stones will develop new stones at some point in their lives. This is why lifelong dietary management and regular veterinary monitoring are not just recommendations but necessities for any guinea pig that has experienced a urinary blockage.
