The typical course of antibiotics for a respiratory infection runs two to three weeks minimum. This is not negotiable. Stopping antibiotics early because your guinea pig looks better is one of the most common and most dangerous mistakes owners make. If using wood shavings, only kiln-dried pine is acceptable, and even then, paper-based alternatives are safer. If you use a cage with solid sides, ensure there is ample open space at the top. Avoid placing cages in enclosed closets, corners with poor air flow, or rooms without windows. At the same time, protect your guinea pig from direct drafts, which stress the respiratory system.

Separate guinea pigs from rabbits. This advice is given frequently because it is critically important. Rabbits commonly carry Bordetella bronchiseptica without showing symptoms and can transmit it to guinea pigs continuously. Even housing them in the same room creates risk through shared airspace. If you keep both species, house them in separate rooms with separate ventilation.

Stress reduction supports immune function and respiratory health. House guinea pigs in compatible pairs or small groups to prevent isolation stress. Provide adequate space, hiding spots, and a consistent routine. Minimize loud noises and sudden environmental changes. Handle your guinea pigs gently and regularly so human interaction is familiar rather than frightening.

Wash your hands before handling your guinea pig, especially during cold and flu season. While transmission from humans is not the most common cause of guinea pig respiratory infections, it is a documented risk factor that is easily mitigated. If you are actively ill with a respiratory infection, consider having another household member handle guinea pig care until you recover, or at minimum, wash hands thoroughly and avoid breathing directly on your pet.

Upper respiratory infections caught early and treated with appropriate antibiotics have a good prognosis. Most guinea pigs show noticeable improvement within three to five days of starting medication and return to normal health by the end of a full antibiotic course. The key factors in this positive outcome are truly early detection, meaning treatment starts when the first subtle symptoms appear rather than after the guinea pig is visibly sick, and completing the full course of prescribed antibiotics without stopping early.

Lower respiratory infections and pneumonia carry a more guarded prognosis. Once bacteria have established themselves in the lung tissue, treatment becomes more difficult and recovery takes longer. Guinea pigs with pneumonia may require extended antibiotic courses of three to four weeks or longer, nebulization therapy, and intensive supportive care. While many guinea pigs with pneumonia do recover, the process is harder on both the patient and the owner, and some guinea pigs develop chronic respiratory issues even after the acute infection resolves.

Chronic respiratory disease is an unfortunate outcome for some guinea pigs, particularly those whose infections were not caught early enough or who have underlying conditions that impair immune function. These guinea pigs may experience recurring bouts of respiratory symptoms, requiring periodic antibiotic treatment and ongoing environmental management to minimize flare-ups.
