Section 1 Overview

Mastitis is an infection and inflammation of the mammary glands, the milk-producing tissue in the chest and abdominal area of guinea pigs. This condition most commonly develops in female guinea pigs that are nursing pups, though it can occasionally occur in non-lactating females and, rarely, in males. Mastitis is a serious condition that causes significant pain and can become life-threatening if the infection spreads to the bloodstream, a condition known as sepsis.

Guinea pigs have two mammary glands, one on each side of the lower belly, each with a nipple. During lactation, these glands are actively producing milk and are particularly vulnerable to bacterial infection. Bacteria can enter through small cracks or wounds on the nipple surface, through the milk duct opening, or from the bloodstream. Once inside the gland tissue, bacteria multiply rapidly in the warm, nutrient-rich environment, causing swelling, pain, and changes to the milk that can be harmful to nursing pups.

The condition requires prompt veterinary treatment because the infection can escalate quickly. A mammary gland that becomes infected can develop an abscess, and bacteria from the infection can enter the bloodstream and spread throughout the body. A nursing mother with mastitis may also stop producing milk or produce infected milk that is dangerous for her pups, creating a secondary crisis of orphaned or underfed babies that need supplemental feeding.

While mastitis is most commonly associated with breeding and nursing, owners of all guinea pigs should be aware of the condition. Non-breeding females can develop mammary gland issues, and any swelling, redness, or sensitivity in the mammary area of any guinea pig should prompt a veterinary examination. Early recognition and treatment provide the best outcomes for both the mother and her pups.

Section 2 Causes

The most common cause of mastitis is bacterial infection of the mammary gland tissue. The bacteria most frequently implicated in guinea pig mastitis include Staphylococcus species, Streptococcus species, Escherichia coli, and Pasteurella species. These bacteria are commonly found in the guinea pig's environment and on the skin, and they become problematic when they gain entry to the mammary gland through a breach in the skin or through the nipple opening.

During lactation, the nipples are constantly exposed to moisture, mechanical friction from nursing pups, and the bacteria present on the pups' mouths and in the bedding. Nursing pups may create small abrasions on the nipple surface with their teeth as they suckle, providing an entry point for bacteria. The nipple opening itself, which is enlarged during active milk production, offers another pathway for bacteria to enter the gland. The combination of these factors makes lactating guinea pigs significantly more vulnerable to mastitis than non-lactating animals.

Poor hygiene in the nursing environment is a major contributing factor. A cage with dirty, damp bedding provides a high bacterial load that is constantly in contact with the mother's mammary area as she lies down to nurse. Bedding that is not changed frequently enough, or bedding that remains wet from water bottle leaks or urine, creates ideal conditions for bacterial growth right next to the vulnerable mammary glands. Keeping the nursing cage scrupulously clean is one of the most effective preventive measures.

Trauma to the mammary glands, whether from nursing pups, rough handling, or contact with sharp cage edges or accessories, can create entry points for infection. Pups with sharp nails can scratch the delicate nipple tissue. Guinea pigs housed on wire-bottom cages or with abrasive cage surfaces are at higher risk of mammary gland irritation. Overly enthusiastic or large litters that nurse vigorously can cause more tissue damage than smaller litters.

Hormonal factors play a role in mastitis susceptibility. The hormonal changes associated with pregnancy, birth, and lactation alter the mammary gland tissue in ways that make it more susceptible to infection. Guinea pigs with hormonal imbalances, such as those with ovarian cysts, may experience mammary gland changes even without pregnancy that can predispose them to mastitis. In rare cases, male guinea pigs can develop mammary gland infections, usually in association with hormonal abnormalities.

Immune suppression from any cause increases the risk of mastitis. A nursing mother that is already stressed from the demands of pregnancy, birth, and lactation has a compromised immune system. If she is also dealing with poor nutrition, vitamin C deficiency, overcrowded housing, or the stress of a difficult social environment, her ability to fight off bacterial invasion of the mammary glands is further reduced. Adequate nutrition and a calm, clean environment during nursing support the mother's immune defenses against mastitis.

Section 3 Symptoms

The most obvious symptom of mastitis is visible swelling of one or both mammary glands. The affected gland appears larger than normal and may feel hard, firm, or lumpy when gently palpated. Compared to the normal soft and slightly enlarged texture of a lactating mammary gland, a mastitic gland feels distinctly different, often harder and more nodular. The swelling may develop relatively quickly, becoming noticeable within a day or two of the infection taking hold.

Redness and warmth of the skin overlying the affected mammary gland are classic signs of inflammation. The skin may appear pink to deep red, and it will feel noticeably warmer than the surrounding areas when you touch it. These signs indicate that the body's inflammatory response is actively fighting the infection, and they distinguish mastitis from simple engorgement or normal lactation-related swelling.

Pain is a prominent feature of mastitis, and the guinea pig will typically show clear signs of discomfort. She may flinch or cry out when the affected area is touched, and she may be reluctant to lie down on the affected side. Nursing pups may cause her visible distress when they attempt to suckle from the infected gland, and she may push them away or avoid nursing altogether. General pain behaviors such as teeth grinding, hunched posture, and reduced activity often accompany the localized mammary pain.

Changes in the milk itself may be apparent if you are able to express a small amount from the affected nipple. Normal guinea pig milk is white to slightly yellowish. Milk from a mastitic gland may appear discolored, watery, thick, clumpy, or blood-tinged. In some cases, pus rather than milk may be expressed from the nipple. These changes in milk quality are dangerous for nursing pups, as infected milk can cause gastrointestinal illness in the babies.

Systemic signs of illness develop as the infection progresses or if bacteria enter the bloodstream. The guinea pig may develop a fever, lose her appetite, become lethargic, and show signs of general malaise. In severe cases progressing toward sepsis, the guinea pig may become very weak, develop rapid breathing, and show signs of shock including cold extremities and pale gums. These systemic signs indicate that the infection has become life-threatening and emergency veterinary care is needed.

If the mastitis progresses to abscess formation, you may feel or see a distinct pocket of fluid within the mammary gland. An abscess may eventually rupture through the skin, releasing pus and infected material. While the rupture itself may provide some relief of pressure and pain, it does not resolve the infection and creates an open wound that requires veterinary management.

Section 4 Diagnosis

Diagnosing mastitis in guinea pigs is typically based on the clinical presentation, with the combination of mammary gland swelling, redness, warmth, pain, and changes in milk or nursing behavior providing strong evidence for the diagnosis. The veterinarian will perform a thorough physical examination, carefully palpating both mammary glands to assess the extent and severity of the infection, checking for abscess formation, and evaluating the guinea pig's overall health status.

Milk samples may be collected for cytology and bacterial culture. Cytological examination of the milk, where a drop is examined under the microscope, can reveal the presence of bacteria, inflammatory cells, and pus cells that confirm infection. Bacterial culture identifies the specific bacteria causing the infection and allows for sensitivity testing, which determines which antibiotics will be most effective against those particular bacteria. This information is especially valuable if the initial antibiotic choice does not produce improvement, as it guides the selection of a more targeted alternative.

Blood work may be recommended, particularly if the guinea pig appears systemically ill. A complete blood count can reveal elevated white blood cell counts indicating active infection, and it can show changes associated with sepsis if the infection has spread to the bloodstream. Blood chemistry can assess organ function and overall metabolic status, which is important for guiding treatment and monitoring recovery.

Imaging may be performed in some cases. Ultrasound of the mammary gland can show the extent of the infection, identify abscess pockets that might not be palpable, and help the vet plan any necessary surgical intervention. X-rays may be taken if the vet suspects the infection has spread beyond the mammary gland or if there are concerns about the guinea pig's respiratory or cardiovascular status.

The vet will also assess the nursing pups if the guinea pig is a lactating mother. The pups may need to be examined for signs of illness from consuming infected milk, and a plan for their nutrition during the mother's treatment will need to be established. This may involve supplemental feeding with a guinea pig milk replacer or, if the pups are old enough, accelerated weaning.

Section 5 Treatment

Treatment for mastitis in guinea pigs centers on aggressive antibiotic therapy to clear the bacterial infection, pain management to keep the guinea pig comfortable, and supportive care for both the mother and her pups. Antibiotics are typically started immediately based on the vet's clinical judgment, with the specific choice guided by knowledge of the bacteria most commonly involved in guinea pig mastitis. If culture results are obtained, the antibiotic may be adjusted based on sensitivity testing. The antibiotic course usually lasts at least two weeks, and it is essential to complete the full course even if the symptoms improve before the medication runs out.

Pain management is critical for the comfort of the guinea pig and for her willingness to eat and care for her pups. Meloxicam or another guinea pig-safe anti-inflammatory medication helps reduce both pain and the inflammatory swelling in the mammary gland. Adequate pain control often improves appetite and activity level noticeably within the first day or two of treatment.

If an abscess has formed within the mammary gland, the vet may need to lance and drain it. This involves making a small incision into the abscess, removing the accumulated pus, and flushing the cavity with antiseptic solution. The wound may be left open to allow continued drainage, or it may be partially closed depending on its size and location. Follow-up wound care at home, including gentle flushing and bandage changes as directed by the vet, supports healing.

The nursing pups' care must be addressed alongside the mother's treatment. If the mastitis is mild and the mother continues to produce milk from the unaffected gland, the pups may continue to nurse from the healthy side while the infected gland heals. If both glands are affected or if the milk is too contaminated, the pups will need supplemental or complete hand-feeding with an appropriate milk replacer. If the pups are close to weaning age, which in guinea pigs is around three to four weeks, accelerated weaning to solid food may be the most practical approach.

In severe cases where the mammary gland tissue is extensively damaged or where the infection is not responding to medical treatment, surgical removal of the affected mammary gland may be necessary. This is a significant surgical procedure but can be life-saving when the infection is severe and refractory to antibiotic therapy. Post-surgical care follows standard recovery protocols for guinea pig surgery, with particular attention to pain management and wound care.

Section 6 Homecare

Home care for a guinea pig with mastitis involves administering medications, managing the nursing situation, and monitoring both the mother and pups closely. Give antibiotics and pain medication exactly as prescribed, maintaining the schedule consistently to keep medication levels steady in the bloodstream. If the vet has prescribed warm compresses for the affected gland, apply them gently for the recommended duration, using a warm, damp cloth held against the area to help reduce swelling and promote drainage.

Keep the nursing cage exceptionally clean during treatment. Change bedding at least daily, and more often if it becomes soiled. Use soft, absorbent bedding that minimizes irritation to the healing mammary area. The cage should be in a quiet, warm location where the mother can rest undisturbed. Reduce handling to the minimum necessary for medication, feeding, and health checks to minimize stress during recovery.

If you are supplemental feeding the pups, establish a consistent feeding schedule and technique. Guinea pig pups are born relatively mature and can often begin eating solid food within the first few days of life, so providing soft hay, small pellets, and finely chopped vegetables alongside any milk feeding helps transition them toward independence. If using a milk replacer, your vet will advise on the appropriate product, dilution, and feeding frequency.

Monitor the affected mammary gland daily, noting any changes in swelling, redness, firmness, or discharge. The gland should gradually become less swollen, less red, and less painful as the antibiotic takes effect. If you notice worsening swelling, new areas of firmness that might suggest abscess formation, increasing pain, or the guinea pig becoming more systemically unwell despite treatment, contact your vet promptly.

Support the mother's nutrition with ample fresh hay, vitamin C-rich vegetables, quality pellets, and fresh water. A nursing and recovering guinea pig needs extra calories and nutrients to support both her healing and any ongoing milk production. Syringe feeding may be necessary if her appetite is suppressed by pain or illness.

Section 7 Prevention

Preventing mastitis begins with creating optimal conditions for nursing mothers and maintaining excellent hygiene throughout the breeding and nursing period. Keeping the cage scrupulously clean is the most direct preventive measure. Change bedding daily during the nursing period, and use soft, absorbent material that stays dry. Wet, soiled bedding harbors the bacteria most commonly responsible for mastitis and keeps them in constant contact with the mother's mammary area.

Provide excellent nutrition to the nursing mother before, during, and after birth. A well-nourished guinea pig with a strong immune system is better equipped to fight off bacterial challenges to the mammary glands. Ensure ample vitamin C through vegetables and supplements, provide unlimited hay for fiber and calories, and offer slightly increased pellet portions during lactation to support the energy demands of milk production.

Monitor nursing pups' nails and trim them if they become sharp enough to scratch the mother's nipple tissue. Small scratches and abrasions from pup nails are a common entry point for bacteria. While pup nails are small and delicate, they can still be gently filed if they are causing visible irritation to the mother.

Watch the nursing process to ensure pups are attaching correctly and that the mother is not showing signs of discomfort during nursing. An awkward latch or overly aggressive nursing can damage nipple tissue and create infection opportunities. If the mother has a large litter, observe whether all pups are getting adequate nursing time and whether the mother appears stressed by the demands.

Regular gentle examination of the mammary glands during the nursing period allows you to catch any early signs of mastitis before the infection becomes severe. Gently feel each mammary gland daily, checking for any new lumps, firmness, warmth, or swelling. Look at the nipples for redness, cracking, or discharge. If you notice any changes, contact your vet promptly rather than waiting to see if they resolve on their own.

For guinea pig owners who are not intentionally breeding, preventing unwanted pregnancies eliminates the primary risk factor for mastitis. Keeping males and females separated, or having males neutered, prevents the pregnancies that make nursing-related mastitis possible. If you do breed your guinea pigs, educate yourself about the signs of mastitis and have a veterinary plan in place before the birth so you can respond quickly if the condition develops.

Section 8 Prognosis

The prognosis for mastitis in guinea pigs depends on the severity of the infection, how quickly treatment is started, and whether the infection remains localized to the mammary gland or spreads systemically. Mild to moderate mastitis that is caught early and treated promptly with appropriate antibiotics generally carries a good prognosis. Most guinea pigs with early-stage mastitis respond well to antibiotic therapy and pain management, with noticeable improvement within the first few days of treatment and full resolution within two to three weeks.

Severe mastitis with abscess formation has a more guarded prognosis but is still often treatable. Abscesses that are successfully drained and treated with appropriate antibiotics typically heal over a period of several weeks. The healing process may require multiple vet visits for wound checks and possible additional drainage procedures. The mammary gland may not return to completely normal function after severe mastitis, which is relevant if the guinea pig is expected to nurse again in the future.

Mastitis that progresses to sepsis, where bacteria have entered the bloodstream and are causing systemic infection, carries a serious prognosis. Sepsis requires intensive veterinary care including aggressive fluid therapy, intravenous antibiotics, and close monitoring. While some guinea pigs do survive sepsis with intensive treatment, the condition is life-threatening and the outcome is not guaranteed. This is why early detection and prompt treatment of mastitis are so important, as preventing progression to sepsis dramatically improves the outlook.

The prognosis for the nursing pups depends on how the mastitis affects their nutrition. Pups that are successfully supplemented with hand-feeding or that transition to solid food early generally do well. Pups that consume infected milk before the condition is recognized may develop gastrointestinal illness, but with supportive care, most recover. The key is monitoring both the mother and the pups closely and intervening with supplemental feeding as soon as mastitis is identified.

Long-term, guinea pigs that recover from mastitis can go on to live healthy, normal lives. If the guinea pig is not bred again, the risk of recurrence is low. If she is bred again, the previously affected mammary gland may be more susceptible to infection during subsequent lactation, and extra vigilance during future nursing periods is warranted. Many veterinarians recommend against breeding a guinea pig that has had severe mastitis, both for her own welfare and to avoid the risk of recurrence.
