Direct transmission from an infested guinea pig is the other major pathway for mange. When a guinea pig without existing mites comes into contact with an infested animal, the mites transfer to the new host. The symptoms of mange mites are among the most dramatic and recognizable of any guinea pig condition. The hallmark symptom is intense, relentless itching that goes far beyond normal scratching. A guinea pig with an active mange infestation will scratch continuously and vigorously, often to the point of creating visible wounds in its own skin. Hair loss typically follows the pattern of the mite infestation, often starting on the rump, back, and shoulders before spreading to other areas. Affected skin beneath the lost hair may appear thickened, crusty, scabby, and inflamed. In severe cases, the skin can develop a rough, leathery texture with extensive scabbing and crusting.
Skin changes are progressive and worsen without treatment. Early in the infestation, the skin may simply appear red and irritated. As the condition advances, the skin becomes thicker and develops the characteristic crusty, scaly appearance. Open wounds from scratching become infected with bacteria, adding additional layers of pain and inflammation. The skin may weep fluid or bleed, and the combination of mite damage and self-inflicted wounds can leave the guinea pig with large areas of raw, damaged skin.
Behavioral changes reflect the guinea pig's suffering. An infested guinea pig may become extremely sensitive to touch, flinching, screaming, or trying to escape when handled, even by familiar owners. Appetite decreases as the constant distress diverts the guinea pig's attention from eating. Weight loss follows reduced food intake. It is worth noting that some guinea pigs, particularly those with mild infestations or strong immune responses, may show more moderate symptoms. Not every case presents with the dramatic seizure-like episodes and severe hair loss described above. A guinea pig with increased scratching, localized hair thinning, and irritated skin patches may have a lower-level mite problem that still needs treatment before it has the chance to escalate.
Section 4 Diagnosis
Diagnosing mange mites typically involves a combination of clinical signs and skin scraping examination. The symptoms of severe mange are distinctive enough that an experienced exotic vet may strongly suspect the diagnosis based on the clinical presentation alone. However, confirming the diagnosis through a skin scraping is considered best practice when possible.
A skin scraping involves the vet using a scalpel blade to gently scrape the surface of the affected skin, collecting cells, debris, and hopefully mites or their eggs. The collected material is placed on a microscope slide and examined under magnification. Trixacarus caviae mites, their eggs, and their fecal matter can be identified microscopically, confirming the diagnosis.
However, it is important to understand that skin scrapings do not always find mites even when they are present. The mites burrow in tunnels within the skin, and a scraping may miss the areas where mites are concentrated. A negative skin scraping does not rule out mange. Many experienced exotic vets will diagnose mange based on clinical signs and treat accordingly even when scrapings are negative. If the guinea pig shows the characteristic symptoms of mange, particularly the intense itching with seizure-like episodes, and responds to antiparasitic treatment, this confirms the diagnosis through therapeutic response.
The vet will also assess the guinea pig's overall condition, checking for secondary bacterial infections in damaged skin, evaluating weight and body condition, and looking for any underlying conditions that may have triggered the mite outbreak. If the guinea pig has lost significant weight or has extensive skin damage, the treatment plan will need to address these secondary issues alongside the mite infestation itself.
Differentiating mange from other conditions that cause itching and hair loss is part of the diagnostic process. Fungal infections, lice, allergic reactions, and other skin conditions can produce some similar symptoms. The severity of the itching, the pattern of hair loss, the characteristic skin changes, and the response to specific treatments all help distinguish mange from these other possibilities. An exotic vet experienced with guinea pig skin conditions will be able to make this distinction efficiently.
Section 5 Treatment
The standard treatment for mange mites in guinea pigs is ivermectin, an antiparasitic medication that is highly effective against Trixacarus caviae. Ivermectin can be administered by injection at the vet clinic or given orally at home. A typical treatment protocol involves multiple doses spaced seven to ten days apart, usually three to four doses total. This spacing is necessary because ivermectin kills adult and larval mites but does not kill eggs. The treatment intervals ensure that mites hatching from eggs after each dose are killed by the next dose before they can lay new eggs.
Revolution (selamectin) is an alternative topical antiparasitic that some vets prefer. It is applied to the skin on the back of the neck and is absorbed to provide systemic parasite control. Your vet will determine the most appropriate medication and dosing schedule based on the severity of the infestation and your guinea pig's overall health.
Pain management is an important component of treating severe mange. The intense skin inflammation and self-inflicted wounds from scratching cause significant pain that needs to be addressed alongside the antiparasitic treatment. Meloxicam or another guinea pig-safe anti-inflammatory and pain medication may be prescribed to provide relief while the mites are being eliminated. Reducing pain helps the guinea pig eat and rest, both of which support recovery.
Secondary bacterial skin infections require antibiotic treatment. The vet will assess whether topical antibiotics applied to infected areas are sufficient or whether systemic oral antibiotics are needed. Keeping the damaged skin clean with gentle antiseptic solutions prescribed by the vet helps prevent new infections from developing during the healing process.
All guinea pigs in the household must be treated simultaneously, regardless of whether they are showing symptoms. Cagemates that appear healthy may be carrying mites at subclinical levels, and leaving them untreated allows them to reinfest the treated guinea pig after the medication wears off. Environmental cleaning, including thorough cage cleaning, bedding replacement, and washing of all fabric items, should be performed at the start of treatment and repeated with each subsequent dose.

Home care during mange treatment focuses on administering medications as prescribed, keeping the environment clean, and supporting your guinea pig's recovery from what has likely been an extremely uncomfortable experience. Handle your guinea pig gently during recovery, understanding that its skin is likely sore and sensitive. Avoid touching areas with active lesions more than necessary. As the treatment begins to work and the itching decreases, you should notice your guinea pig becoming more tolerant of handling and more comfortable in general. This return of normal touch tolerance is a positive sign of progress.
Encourage eating by offering fresh, appealing foods at regular intervals. Hay, vitamin C-rich vegetables, and small amounts of favorite healthy treats may tempt a guinea pig whose appetite has been suppressed by the discomfort of the infestation. If independent eating remains insufficient, continue syringe feeding to maintain nutritional status and gut function. Monitor weight to track whether nutritional intake is adequate for recovery.
Watch for signs of improvement as treatment progresses. Reduced scratching, improved appetite, increased activity, and a calmer demeanor are all positive indicators. Skin healing, with scabs falling off and new skin forming underneath, usually becomes visible within two to three weeks of starting treatment. Hair regrowth follows skin healing and may take several additional weeks to become apparent.

Stress reduction supports immune function and helps prevent the mite population explosions that cause symptomatic mange. Provide your guinea pigs with a stable, predictable environment that includes adequate cage space, compatible companionship, hiding places for security, consistent routines, and protection from loud noises and disturbances. Manage transitions such as new introductions, cage moves, and environmental changes carefully and gradually.
Quarantine all new guinea pigs for two to three weeks before introducing them to your existing pets. During quarantine, observe the new guinea pig for any signs of scratching, hair loss, or skin irritation. A veterinary examination during the quarantine period can identify mites and other parasites before they have a chance to spread to your established guinea pigs. Part the fur and look at the skin during your regular handling and bonding time. The prognosis for guinea pigs with mange mites is excellent when the condition is properly diagnosed and treated. The speed of hair regrowth depends on the severity of the damage and the individual guinea pig's healing capacity. Some guinea pigs regrow a full coat relatively quickly, while others may have permanent thin patches in areas where the hair follicles were severely damaged.
The seizure-like episodes that accompany severe mange resolve completely once the mite burden is reduced and the skin inflammation subsides.
