However, normal-appearing bullae on X-ray do not completely rule out ear infection, as early or mild infections may not produce visible bony changes.
CT or MRI scanning provides much more detailed images of the ear structures, brainstem, and brain than standard radiographs. These advanced imaging modalities can detect middle and inner ear infections, abscesses, tumors, and brain lesions that may not be visible on regular X-rays. Availability of CT and MRI for guinea pigs depends on the facilities in your area, and these procedures require general anesthesia.
Blood work and culture testing may be performed to identify the bacteria involved in an infection and determine which antibiotics will be most effective. If ear discharge is present, a culture of this discharge can be particularly helpful for targeting antibiotic therapy.
Section 5 Treatment
Treatment for head tilt in guinea pigs is directed at the underlying cause while also providing supportive care to manage the symptoms and maintain the guinea pig's health during recovery.
Antibiotic therapy is the cornerstone of treatment when infection is the suspected or confirmed cause, which is the most common scenario. Your vet will select a guinea pig-safe antibiotic based on the likely bacteria involved and may adjust the choice if culture results suggest a different antibiotic would be more effective. Section 6 Homecare
Home care for a guinea pig with head tilt focuses on safety, medication administration, nutritional support, and creating an environment that accommodates their impaired balance.

Reduce the risk of head trauma by providing a safe living environment. Ensure the cage has smooth, solid flooring. Remove objects that guinea pigs could fall from. Handle your guinea pig gently and securely, and teach all family members proper handling techniques. Sit down when holding guinea pigs to prevent injury from drops.

GI stasis developing as a complication of head tilt is one of the biggest threats to survival, which is why nutritional support is such a critical component of treatment.
Head tilt caused by tumors has a more guarded prognosis that depends on the type, location, and treatability of the growth. Some tumors can be managed for a period, while others are progressive and eventually require end-of-life decisions.
Even guinea pigs with permanent head tilt can often have a good quality of life. Guinea pigs are remarkably adaptable, and many learn to compensate for a persistent tilt over time. They adjust their movement patterns, learn to navigate their familiar environment, and continue to eat, interact, and engage in normal guinea pig behaviors. Providing a safe, adapted living space supports this adjustment process.
