Eye infections are among the most common health problems guinea pig owners encounter, and they can range from mild conjunctivitis that clears up quickly with treatment to serious conditions that threaten your guinea pig's vision if left unchecked. An eye infection occurs when bacteria, and less commonly fungi or viruses, invade the tissues of the eye or the surrounding structures. The result is inflammation, discharge, pain, and if untreated, potential damage to the delicate structures that allow your guinea pig to see.
Guinea pigs are especially vulnerable to eye infections for several reasons related to their anatomy and lifestyle. Their large, prominent eyes protrude slightly from the skull, giving them excellent peripheral vision for detecting predators but also exposing more of the eye surface to potential irritants and pathogens. Living at ground level means their eyes are constantly close to bedding, hay dust, and other debris that can introduce bacteria. Even a microscopic scratch on the cornea from a piece of hay can provide an entry point for infection.
What makes eye infections in guinea pigs particularly concerning is how quickly they can escalate. A guinea pig's immune system handles infections differently than a dog's or cat's, and what appears to be a minor irritation one day can become a swollen, painful, pus-filled infection within 24 to 48 hours. Guinea pigs are also prey animals that instinctively hide signs of illness, which means by the time you notice obvious symptoms, the infection may already be well-established.
The encouraging aspect of guinea pig eye infections is that most respond very well to appropriate treatment when caught early. The vast majority of bacterial eye infections clear up completely with the right antibiotic eye drops, and even more serious infections can usually be managed effectively with prompt veterinary care. The key is recognizing the early signs, getting your guinea pig to an exotic vet quickly, and following through with the full course of prescribed treatment.

Bacterial infection is by far the most common cause of eye infections in guinea pigs. The bacteria involved are often opportunistic pathogens that are normally present in the environment but only cause problems when they find a way past the eye's natural defenses. Staphylococcus, Streptococcus, Bordetella, and Pseudomonas are among the most frequently identified bacterial culprits. These bacteria can enter the eye through tiny abrasions on the corneal surface, through contaminated discharge from the nose via the tear ducts, or simply by overwhelming the eye's natural protective barriers when conditions favor bacterial growth.
Physical injury to the eye is one of the most common triggers for infection. Guinea pigs frequently get poked in the eye by hay stalks, which is nearly impossible to prevent entirely since hay is an essential part of their diet. Rough or dusty bedding can scratch the cornea, and even vigorous scratching with a toenail can damage the eye surface. These injuries may be too small to see with the naked eye, but they create openings where bacteria can gain a foothold and multiply. What starts as a minor scratch can become a full-blown infection within a day or two.
Environmental conditions play a major role in how likely eye infections are to develop. Dirty cage conditions allow bacteria to thrive in bedding, food dishes, and water bottles. Ammonia from accumulated urine irritates and damages the eye surface, weakening its natural defenses. High humidity combined with poor ventilation creates conditions where bacteria flourish. Dusty bedding and hay not only cause mechanical irritation but also carry bacterial particles directly to the eye surface.
Dental disease is an often-overlooked cause of eye infections in guinea pigs. The roots of the upper back teeth sit very close to the eye socket and tear ducts. When these teeth become overgrown, a condition called malocclusion, the elongated roots can compress or block the tear ducts. Normal tear drainage is disrupted, creating a pool of stagnant fluid that becomes a breeding ground for bacteria. Guinea pigs with dental disease may experience recurring eye infections that keep coming back despite proper treatment because the underlying dental problem is never addressed.
Respiratory infections frequently lead to secondary eye infections. The nasolacrimal duct connects the eye to the nasal passages, and when a guinea pig has an upper respiratory infection, bacteria can travel through this duct in both directions. A guinea pig being treated for a respiratory infection may develop eye symptoms as the bacteria spread, or conversely, what appears to be a simple eye infection may actually be the first visible sign of a developing respiratory illness.
Weakened immune function from stress, poor diet, or concurrent illness increases susceptibility to eye infections. Guinea pigs that are deficient in vitamin C have compromised immune systems and heal more slowly from any type of infection. Overcrowded living conditions create stress that suppresses immune function while simultaneously increasing exposure to pathogens. Young guinea pigs with immature immune systems and elderly guinea pigs with declining immune function are both at higher risk than healthy adults in their prime.
Section 3 Symptoms
The earliest signs of an eye infection are often subtle enough that they can be missed if you are not looking closely. You might notice your guinea pig blinking more frequently than usual on one side, or the eye may appear slightly wetter than normal. A small amount of clear, watery discharge might be present at the inner corner of the eye. The guinea pig's behavior may be completely normal at this stage, which is why daily visual checks of your pet's eyes are so important for catching problems before they progress.
As the infection develops, the symptoms become more obvious and harder to miss. The discharge typically changes from clear to white, then to yellow or green as the immune system responds to the bacterial invasion. This colored discharge is pus, which consists of white blood cells, dead bacteria, and tissue debris. You may notice the discharge crusting around the eye, particularly after your guinea pig has been sleeping. The fur below the eye may become matted and damp, and in light-colored guinea pigs, you might see staining of the fur in the discharge path.
The affected eye itself shows visible signs of inflammation. The conjunctiva, the pink membrane lining the eyelids, becomes red and swollen. This redness may be obvious when you gently pull down the lower eyelid. The eyelids themselves may appear puffy, and in more serious infections, the swelling can be significant enough to partially or fully close the eye. The eye may look cloudier than normal if the infection has affected the cornea.
Pain behaviors are important indicators that help distinguish a simple irritation from a true infection. A guinea pig with an eye infection will often squint or hold the affected eye partially closed. They may paw at the eye or rub their face against bedding and cage surfaces. Some guinea pigs become head-shy, flinching or pulling away when you try to look at or touch the area around the affected eye. Light sensitivity may develop, causing the guinea pig to prefer darker areas of their enclosure.
Behavioral changes beyond the eye itself often accompany infection. Many guinea pigs become quieter and less active than usual. Appetite may decrease, partly from the pain and discomfort and partly because guinea pigs that feel unwell instinctively reduce food intake. You may notice your guinea pig hiding more than usual or being less responsive to interaction. These are signs that the infection is affecting their overall wellbeing, not just the eye.
Severe or advanced eye infections present with alarming symptoms that demand emergency veterinary care. A completely swollen-shut eye, especially with colored discharge oozing from between the lids, indicates a serious infection that could damage the eye permanently. A cloudy or opaque cornea suggests the infection has penetrated deeper structures. Any visible ulceration or crater on the eye surface is an emergency. A bulging eye could indicate an abscess forming behind the eyeball. If you see any of these signs, do not wait for a regular appointment. Seek emergency exotic veterinary care immediately.
Section 4 Diagnosis
An exotic veterinarian will approach diagnosing an eye infection through a systematic examination that looks at both the eye itself and the potential underlying causes. The visit begins with a thorough history where your vet asks when symptoms started, how they have progressed, whether one or both eyes are affected, and what your guinea pig's living conditions are like. They will want to know about bedding type, cage cleaning routine, diet, and whether any cage mates are showing similar symptoms.
The eye examination uses specialized tools to get a close look at all the structures involved. Your vet will examine the eye surface under magnification, checking for scratches, ulcers, foreign bodies, and signs of deeper infection. A fluorescein stain test is standard for eye infections. This involves placing a drop of orange-green dye on the eye surface, which adheres to any area where the corneal surface is damaged. Under blue or ultraviolet light, these damaged areas glow bright green, revealing injuries that are invisible to the naked eye. This test is painless and provides critical information about whether a corneal ulcer is present.
A Schirmer tear test may be performed to measure tear production. This involves placing a small strip of special paper inside the lower eyelid for a set time and measuring how much moisture it absorbs. Abnormal tear production can both contribute to and result from eye infections. Your vet will also assess the intraocular pressure if glaucoma is a concern, using a device called a tonometer that gently touches the corneal surface.
If the infection appears bacterial, which most are, your vet may take a swab of the discharge for culture and sensitivity testing. This lab test identifies the specific bacteria causing the infection and tests which antibiotics are most effective against it. Results typically take two to five days, so your vet will usually start treatment with a broad-spectrum antibiotic while waiting for results and may adjust the medication later based on the culture findings.
Because eye infections can be secondary to other conditions, your vet will likely examine beyond just the eye. An oral exam checks for dental disease that could be affecting the tear ducts. Listening to the lungs and checking for nasal discharge assesses for respiratory infection. If the infection is severe, recurrent, or not responding to treatment, additional diagnostics such as skull X-rays, blood work, or a CT scan may be recommended to identify hidden contributing factors.
Section 5 Treatment
The cornerstone of treating bacterial eye infections in guinea pigs is topical antibiotic medication applied directly to the eye. Your vet will prescribe either antibiotic eye drops or eye ointment based on the specific situation. Drops are easier to apply and do not blur vision as much, but they need to be applied more frequently, typically three to four times daily. Ointment stays on the eye longer and may only need application two to three times daily, but it can be harder to apply accurately. Common antibiotic choices include chloramphenicol, ciprofloxacin, ofloxacin, fusidic acid, and gentamicin.
For more serious infections, particularly those involving corneal ulcers, treatment may be more aggressive. Your vet might prescribe multiple eye medications to be used together, such as an antibiotic combined with an anti-inflammatory drop. In cases of deep corneal ulcers, atropine drops may be added to dilate the pupil and reduce painful spasm of the muscles inside the eye. Some severe ulcers require more frequent medication application, sometimes every two hours initially, tapering to less frequent dosing as the ulcer begins to heal.
Systemic antibiotics, given orally, are added to the treatment plan when the eye infection is part of a broader problem such as a respiratory infection or when the infection is severe enough that topical treatment alone may not be sufficient. Pain medication is an important component of treatment that is sometimes overlooked. Eye infections are painful, and managing that pain helps your guinea pig feel better, eat normally, and recover faster. Your vet may prescribe meloxicam or another guinea pig-safe pain reliever.
If the eye infection is secondary to dental disease, treating the teeth is essential for resolving the eye problem long-term. This may involve dental procedures under anesthesia to file down overgrown molars or extract problematic teeth. Tear duct flushing may be performed to clear blockages caused by dental root impingement. Without addressing the dental component, the eye infection will likely recur.
Followup care is a critical part of treatment. Your vet will typically want to recheck the eye within five to seven days of starting treatment, or sooner if the infection is severe. The fluorescein stain test may be repeated to track healing of any corneal damage. If the infection is not responding as expected, the treatment plan may be adjusted based on culture results or clinical findings.
Section 6 Homecare
Successfully treating an eye infection at home requires consistent medication administration and a supportive environment. Before applying any eye medication, gently clean away discharge and crusting with a warm, damp cotton pad or soft cloth. Use clean, warm water or sterile saline solution. Wipe from the inner corner of the eye outward, and always use a fresh pad for each eye to prevent cross-contamination. This cleaning step is essential because medication applied on top of discharge cannot reach the eye surface effectively.
To apply eye drops, wrap your guinea pig snugly in a small towel to prevent squirming. Hold them securely and gently tilt the head slightly upward. Use one hand to hold the upper eyelid open while bringing the dropper in from the side with your other hand. Squeeze one drop onto the eye surface, being careful not to touch the dropper tip to the eye. For ointment, pull down the lower eyelid slightly and place a small ribbon of ointment along the inside of the lid. The guinea pig's blinking will spread it across the eye. Give a small treat immediately afterward to build a positive association.
Environmental modifications during treatment help the eye heal and prevent reinfection. Switch to the lowest-dust bedding available, or use fleece liners if possible. Clean the cage more frequently than usual, ideally spot-cleaning twice daily and doing full changes every three to four days. Remove any sharp objects or rough surfaces near where your guinea pig sleeps that could irritate the healing eye. If your guinea pig has cage mates, watch for any rough play that could injure the affected eye.
Monitoring progress is your most important job during home treatment. Check the eye at each medication time and note whether the discharge is decreasing, the swelling is going down, and the eye is opening more fully. Take photos daily to track changes since gradual improvement can be hard to notice in real time. If you see no improvement after three days, the infection is getting worse, or new symptoms develop, contact your vet immediately rather than waiting for the scheduled recheck.
Section 7 Prevention
Preventing eye infections begins with maintaining a clean living environment that minimizes your guinea pig's exposure to bacteria and irritants. Choose low-dust or dust-free bedding and keep the cage on a regular cleaning schedule. Spot-clean soiled areas daily and do a complete bedding change at least weekly, more often if you have multiple guinea pigs sharing the space. Ammonia buildup from urine is particularly damaging to the eyes and respiratory system, so never let bedding become saturated. Ensure good ventilation in the room where your guinea pig lives without creating direct drafts on the enclosure.
Hay management is an important but often overlooked aspect of eye infection prevention. While unlimited hay is non-negotiable for guinea pig health, how you present it can reduce eye irritation. Hay racks and bags that hold hay at a comfortable eating height keep your guinea pig from pushing their face deep into loose piles where dust and sharp stalks can contact the eyes. Shake out or sift new hay to remove the dustiest particles. If you notice a particular brand or cut of hay seems especially dusty or poky, consider switching to another source.
Keeping your guinea pig's immune system strong is fundamental to infection prevention. A diet rich in vitamin C is essential since guinea pigs cannot produce their own and deficiency weakens immune function significantly. Fresh bell peppers, leafy greens, and other vitamin C-rich vegetables should be part of the daily diet. Unlimited timothy hay supports dental health, which in turn protects the tear ducts from compression by overgrown tooth roots. Minimize stress by providing adequate space, hiding spots, a consistent routine, and appropriate social companionship.
Regular observation is your best tool for catching potential eye infections before they become serious. Make it a daily habit to look at both of your guinea pig's eyes during handling or feeding time. They should be equally bright, clear, open, and free of discharge. Any change from this baseline warrants closer monitoring over the next day or two, with a vet visit if symptoms persist or worsen. Early detection and treatment of eye infections is consistently associated with faster recovery, lower treatment costs, and better outcomes for your guinea pig's vision and overall health.
Section 8 Prognosis
The prognosis for most guinea pig eye infections is very good when they are diagnosed and treated promptly. Simple bacterial conjunctivitis, the most common type of eye infection, typically shows noticeable improvement within two to three days of starting appropriate antibiotic treatment and resolves completely within seven to fourteen days. The guinea pig usually experiences no lasting effects on their vision or eye health, and with proper environmental management, recurrence can be prevented.
Corneal ulcers have a more variable prognosis depending on their depth and severity. Superficial scratches and small ulcers generally heal well within one to two weeks with proper treatment, leaving little to no scarring. Deeper ulcers take longer to heal and carry a greater risk of scarring that could affect vision in the affected area. In rare cases, a deep corneal ulcer can perforate, which is a serious complication requiring emergency treatment and potentially resulting in the loss of the eye. The vast majority of corneal ulcers caught and treated early, however, heal without major complications.
Eye infections secondary to respiratory illness have a prognosis that is tied to the respiratory infection itself. If the respiratory infection responds well to antibiotics, the eye symptoms typically resolve along with it. More stubborn respiratory infections may require longer courses of treatment or changes in antibiotics, and the eye symptoms will persist until the underlying infection is controlled. In severe respiratory cases, the overall prognosis is more guarded, though the eye component is rarely the determining factor.
Recurrent eye infections caused by dental disease present the most challenging long-term picture. While each individual episode of infection can be treated successfully, the underlying dental problem usually means the infections will return. Many guinea pigs with dental-related eye issues require ongoing management, including regular dental procedures and periodic courses of antibiotic eye medication. Despite this chronic nature, most guinea pigs with dental disease maintain a good quality of life with attentive care.
Overall, the most important factor in the prognosis of any guinea pig eye infection is how quickly treatment begins. Guinea pigs whose owners notice symptoms early and seek veterinary care promptly have significantly better outcomes than those where the infection has been present for days or weeks before treatment starts. This is why developing the habit of daily eye checks and not taking a wait-and-see approach with eye symptoms is so valuable for your guinea pig's long-term eye health.
